Responsibilities
The Choice Chaplain is responsible for assessing, providing, and meeting the spiritual care in the home to the terminally ill patients and their families
Makes reasonable efforts to facilitate visits by local clergy, pastoral counselors or other individuals who can support the patient's spiritual needs
Participates as a core member of the Interdisciplinary Group
As a Hopestone Hospice Chaplain, You Will
Assess hospice patients and support systems to identify spiritual needs as evidenced by POC, documentation, clinical records, and IDG conference minutes
Provide support for the spiritual needs of the patient/family
Attend interdisciplinary team meetings and participate in the patient care planning process
Use effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes
Meet mandatory continuing education requirements of the agency
Demonstrate commitment, professional growth and competency by maintaining a working knowledge of public and private eligibility standards and requirements
Promote Agency philosophy and administrative policies to ensure quality of care
Demonstrate understanding of coordination and continuity of care by collaborating with the IDG
Accept clinical assignments that are consistent with education and competence to meet the needs of the patients/families

Position Summary
The Hospice Chaplain has a true calling and is a passionate advocate for the patients we serve. The Choice Chaplain is responsible for assessing, providing, and meeting the spiritual care in the home to the terminally ill patients and their families. Makes reasonable efforts to facilitate visits by local clergy, pastoral counselors or other individuals who can support the patient's spiritual needs. Requirements
• One year of experience as a Chaplain in health care preferred
• One or more years of Hospice experience required
• CPR Certification
• Valid Driver's License
• Valid Auto Insurance
• HomeCare HomeBase experience preferred
